Students seeking accommodations must submit current documentation of a disability to Accessibility Services. Acceptable documentation may include evaluations or letters from a qualified professional, as well as prior Individualized Education Programs (IEPs) or 504 Plans dated within the past five years. Documentation must be provided on the provider’s official letterhead or professional document template and include the provider’s name, office address, phone number, and professional license number. Documentation must be dated within the last five years and include the following information:
Note: Accommodations provided cannot fundamentally alter the nature of the course work. Reasonable accommodations are determined through an interactive process based on each student’s individual needs, documentation, and the essential requirements of their courses.

You may bring your documentation to your scheduled appointment or email it to the Accessibility & Wellness Office in advance.
You will meet with a member of the Accessibility & Wellness team to discuss your learning needs and identify appropriate accommodations to support your academic success.
Once accommodations are approved, the Accessibility & Wellness Office will notify your instructors in writing.
Students are encouraged to stay engaged in the accommodation process and to communicate with both the Accessibility & Wellness Office and their instructors as accommodations are used or adjustments are needed throughout the semester. Appointments are recommended, but walk-ins are also welcome. When you arrive, please check in at the Student Services One Stop Desk. A member of the Accessibility Services team will meet you there.
